We understand that having any type of healthcare procedure can be overwhelming. If you are scheduled for an Outpatient procedure at Cannon Memorial Hospital, providing the following information will help in making your experience as pleasant and prompt as possible.
Pre-Register and Save Time
If your physician has scheduled you for a procedure, please call 898-1242, Monday-Friday between 8:30am and 5:00pm to pre-register before your appointment. This will allow us to meet your insurance pre-certification requirements and also decrease your wait time.
Information Needed for Pre-Registration:
– Name, Address & Phone number
– Social Security Number
– Date of Birth
– Insurance Card information
– Employer Information
– Guarantor Information
– Date of Procedure
– Doctors Name
– Type of Procedure
Information needed when you arrive
Please make sure you bring the following to your procedure:
– Doctor’s order
– Insurance Information
– Photo ID
Instructions the Day of Your Procedure
When you arrive for your appointment and you have already pre-registered, please sign in at our outpatient department (click here for outpatient parking information) and tell the receptionist that you have already pre-registered. We will then make a copy of your insurance card and have you to sign your consent. You will then be directed to the correct department. Our staff is required to collect any co-pays or deductibles at the time of registration.
